I understand getting a tattoo will permanently alter my skin; because of the nature of anatomy and the passage of time the design may have variations in colour and shape

All precautions have been taken by Inkwell Tattoos and it's artists however infection is always possible. Following the artists' aftercare instructions will greatly reduce the chances of this happening. It is my responsibility to take care of my new tattoo and I agree to pay for any touch ups if need be.

I consent to Inkwell Tattoos and their artists' to use photos and videos of the procedure and or the finished product on a prediscussed basis with the artist.

I consent to the procedure and accept all risks and responsibilities relating to my body modification procedure. I have read and understood this form and waive and release Inkwell Tattoos and all artists from all liabilty concerning the same.
